Participant Waiver and Release of Liability
1. Assumption of Risk
I, the undersigned parent or legal guardian, acknowledge that participation in Blades Power Skating programs involves inherent risks, including but not limited to falls, collisions with other skaters or boards, equipment failure, and other hazards associated with ice skating and hockey training. I understand that these risks may result in injury, illness, or in rare cases, serious harm. I voluntarily assume all such risks on behalf of my child(ren).
2. Waiver and Release
In consideration of participating in Blades Power Skating, I represent that I understand the nature of power skating clinics and that the player is qualified and in proper physical condition to participate. I fully understand that hockey involves risks of serious bodily injury. I hereby release and discharge Blades Power Skating from all liability or claims. I agree to indemnify and hold harmless Blades Power Skating, its owners, employees, coaches, and volunteers from any and all claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury that may be sustained by my child(ren) while participating in Blades Power Skating programs. This waiver and release of liability extends to all claims of every kind or nature whatsoever, including but not limited to those caused by the negligence of Blades Power Skating or its representatives. I understand that this agreement is binding on me and my heirs, assigns, and legal representatives.
3. Medical Authorization
I authorize Blades Power Skating staff to seek and obtain emergency medical treatment for my child(ren) in the event of an injury or medical emergency during a session, if I am not immediately available. I understand that I am responsible for all costs associated with any medical treatment provided.
4. Equipment Requirement
I understand that full hockey gear, including a HECC-approved helmet and neck guard, is strictly required for all participants at every session. Blades Power Skating reserves the right to deny participation to any skater who arrives without proper equipment, with no refund issued.
5. Media Release
I grant Blades Power Skating permission to photograph or video record my child(ren) during sessions for use in promotional materials, social media, and the Blades Power Skating website. No names will be associated with any images without separate written consent.
6. Refund Policy
I understand that all registration payments are final. There are no refunds, credits, or transfers once payment is processed. Session dates and times are subject to change, and Blades Power Skating will make reasonable efforts to communicate any changes in advance.
7. Code of Conduct
I agree that my child(ren) will follow the instructions of all coaching staff, treat other participants with respect, and maintain appropriate behavior during sessions. Blades Power Skating reserves the right to remove any participant whose behavior is disruptive or unsafe, without refund.
8. Agreement
By signing below, I confirm that I am the parent or legal guardian of the participant(s), that I have read and understand this waiver in its entirety, and that I agree to all terms and conditions outlined above. I am signing this waiver voluntarily and acknowledge that it is binding.
